Nursing Home Transition and Diversion Program

It is the goal of All Metro Health Care’s Nursing Home Transition and Diversion Program to provide every individual, for which the Company cares, the opportunity to return to the community and receive the complex medical services and support they need in the comfort and familiar surroundings of their home. 

New York State’s NHTD Medicaid waiver was developed based on the philosophy that individuals with disabilities and/or seniors have the same rights as others.  This includes the right to be in control of their lives, encounter and manage risks and learn from their experiences.  This is balanced with the waiver program’s responsibility to assure the waiver participant’s health and welfare.

Waiver services are provided based on the participant’s unique strengths, needs, choices and goals.  The individual is the primary decision maker and works in cooperation with All Metro Health Care to develop a Service Plan.  This process leads to personal empowerment, increased independence, greater community inclusion, self-reliance and meaningful, productive activities.  Waiver participant satisfaction is a significant measure of success of the NHTD waiver.

All Metro Health Care is an approved provider of NHTD services by the New York State Department of Health (NYSDOH).   All Metro Health Care is approved to provide the following services: (i) Service Coordination; (ii) Home and Community Support Services (HCSS); and (iii) Respite Services.

Service Coordination
The Service Coordinator functions as the client’s advocate in the community. He or she assists the prospective client with the enrollment process and coordinates and monitors the provision of all services in the Service Plan.  Services may include Medicaid State Plan services, non-Medicaid federal, state and locally funded services, as well as educational, vocational, social, and medical services. The Service Coordinator’s goal is to increase the client’s independence, productivity and integration into the community while maintaining the health and welfare of the individual.

Home and Community Support Services
Home and Community Support Services (HCSS) are available on an hourly basis, and are individually customized to provide prompts, direction, supervision, and support in the following areas: (i) personal care needs; (ii) maintaining a clean and safe home; (iii) planning and completion of shopping; (iv) planning and completion of meals; (v) managing appointment and keeping appointments; (vi) traveling within the community; (vii) identifying social activities; and (viii) protective oversight and emergency assistance. 

After services begin, periodic home supervisory visits are conducted by Registered Nurses to assure the services are being provided as desired and to assure consumer satisfaction.  Furthermore, tasks performed by the HCSS worker(s) are documented each day and are made available to the consumer’s physician, authorized family members, and service coordinator.  In the event that an issue or question arises, the company’s office staff and nursing supervisors are available 24 hours a day, 7 days a week, 365 days a year

Respite Services
Respite Services are individually designed services intended to provide relief to natural (informal), non-paid supports who provide primary care and support to a waiver participant. This is usually provided for participants who are in need of oversight and supervision as a discrete task. The primary location for the provision of this service is the waiver participant's home. Respite Services are provided in a 24 hour block of time.
